Civilian Accident Report
If you have been involved in an accident in which you have moved the vehicles before the police arrived you will need to fill out a Civilian Accident Report form and submit it to the Kentucky State Police as well as your insurance company. The Civilian Accident Report can be downloaded here:

Vacation Patrol Request
If you are going to be away from your residence for a period of time and would like to notify the police officers to keep an eye on your home, you may fill out a Vacation Patrol Request form here:
